Job Summary:

Reentry Case Manager (RCM) will assist all clients of Gastineau Human Services (GHS) to resolve problems in housing, employment, education, transportation, substance abuse treatment, resolve legal issues, improve mental and physical health, resolve family and social issues and develop healthy life skills.

The RCM will work with community partners to assist the clients to access necessary services and benefits. The RCM will assist clients to fulfill their obligations to referral agencies such as Department of Corrections, Office of Children’s Services, court programs while promoting appropriate levels of autonomy and self-determination.

As a client advocate, the RCM will engage in respectful, compassionate, fair interactions with clients from diverse backgrounds and help to remove obstacles that impair their ability to achieve their goals.  RCMs must have flexible schedules that allow them to provide the highest degree of contact with clients of GHS.

 Duties/Responsibilities:

  • Provides intensive case management services during times when most clients are available: referrals, advocacy, coordination of services, and collaboration with other staff and outside service providers to meet client needs.
  • Works with all GHS Clients from entry to discharge.
  • Maintains detailed and accurate case management files and records, intake paperwork, client census, and inputs client information to required data bases.
  • Prepares timely and accurate monthly summaries for each client on caseload for review by the Re-Entry Services Director.

Education and Experience:

  • High School diploma or GED.
  • At least one year of experience in direct human services

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must pass a DOC and DOH background check.
  • Must have a current driver’s license and be insurable by current GHS auto insurance carrier.
  • Must pass drug test upon hire.

Schedule and Pay Range:

  • Schedule: 40 hours per week, M-F; with at least one evening shift.
  • Hourly: $22.28 to $37.13 (Annual: $46,340 – $77,234)
